Mental health and psychological well-being: Towards an integrated system of care, prevention, and promotion

Abstract

The growing demand for psychological support in contemporary societies highlights the need to rethink current mental health systems. This paper examines the implications of the Italian National Action Plan for Mental Health 2025-2030, emphasizing its shift toward a broader and multidimensional conception of mental health. Within this framework, psychological well-being is conceptualized not merely as the absence of disorder, but as a structural component of overall functioning and a key determinant of physical health, quality of life, and social participation.

Drawing on international evidence, the paper argues that current service models ‒ largely focused on severe disorders, acute care, and pharmacological treatment ‒ are only partially adequate to address emerging needs, which are increasingly widespread, less categorical, and deeply embedded in everyday life contexts. Particular attention is given to the “intermediate area” between well-being and mental illness, where many forms of contemporary distress arise and often remain insufficiently addressed.

The paper proposes a systemic perspective based on the integration of two complementary networks: a mental health care system focused on diagnosis and treatment, and a broader psychological well-being system aimed at prevention, promotion of personal resources, and support within community settings. In this view, psychological well-being is framed as an “intangible infrastructure” underpinning health, social cohesion, and economic sustainability.

The transition from an illness-centered to a multi-level, integrated model is presented as a necessary step to respond effectively to current challenges. Strengthening psychological competencies within public systems and everyday contexts is therefore not only a clinical priority, but also a strategic investment in the resilience and sustainability of societies.
