Il craving si riferisce a un'esperienza soggettiva di desiderio intenso, un'urgenza di usare una sostanza o di impegnarsi in un comportamento [Drummon et al., 2001, Drummond et al. 2002]. Attualmente, il craving è riconosciuto come uno dei criteri principali per la diagnosi dei disturbi correlati a sostanze e dei disturbi da dipendenza (DSM5, Apa, 2013), incluso nel raggruppamento di criteri relativi alla compromissione del controllo dell'uso di sostanze ed è un fattore centrale nel mantenimento della dipendenza (Hasin et al., 2013; Romanczuk-Seiferth, Van Den Brink e Goudriaan, 2014). Secondo il DSM-5, il craving può essere definito come un'"urgenza o desiderio intenso" prontamente stimolato da fattori interni ed esterni precedentemente associati alla sostanza, elementi in grado di svolgere un ruolo di "trigger", ovvero di "innesco", che innesca, attraverso un meccanismo di condizionamento e per associazione di idee, il desiderio di gratificazione ottenuta chimicamente (Tiffany e Conklin, 2000). Due forme distinte di craving dal punto di vista delle aspettative del paziente: da un lato, la preoccupazione di assumere la sostanza per evitare l'astinenza, definita "craving negativo"; dall'altro, la compulsione verso la sostanza guidata dall'aspettativa di incentivi, di gratificazione. In questo caso, la ricerca di una "ricompensa" produrrebbe un "craving positivo". (Tiffany e Conklin, 2000) La ricerca ha dimostrato che il craving è qualitativamente simile in tutti i domini target, inclusi alcol e tabacco (Castellani e Rugle, 1995; Field, Schoenmakers e Wiers, 2008). Negli ultimi 40 anni, sono stati proposti diversi modelli per concettualizzare il craving; i modelli comportamentisti lo hanno concettualizzato come un epifenomeno dei processi di condizionamento classico e operante (ad esempio, Ludwig e Wikler, 1974; Stewart, Dewit e Eikelboom, 1984), mentre i modelli cognitivi hanno sottolineato che il funzionamento cognitivo di ordine superiore e il modo in cui vengono elaborate le informazioni sono centrali nell'attivazione del craving (Tiffany, 1999). Tra i costrutti cognitivi proposti negli ultimi anni vi è quello del "pensiero del desiderio" o pensiero di volere, che sembra svolgere un ruolo nell'escalation e nella persistenza del craving negli individui dipendenti (Caselli & Spada, 2010, 2011; Kavanagh et al., 2009). Lo scopo di questo breve articolo è quello di stimolare, tra i clinici che lavorano con pazienti affetti da disturbi da uso di sostanze (SUD), una riflessione sulle analogie esistenti tra il costrutto e il meccanismo d'azione del "pensiero del desiderio" e quello della "tentazione" così come teorizzato dai Padri della Chiesa. L'analogia e il paragone tra tentazione e pensiero del desiderio vanno qui intesi come del tutto scevri da connotazioni etico-morali e/o religiose e considerati solo ed esclusivamente come un confronto tra due costrutti cognitivo-emotivi ben definiti. Nei resoconti narrativi dei pazienti, sia laici che cattolici, raccolti da terapeuti clinici (sia laici che cattolici), le espressioni "tentazione" e "liberazione" relative al craving sono spesso presenti. Alla fine dell'articolo discuteremo la possibilità di modificare queste strategie di coping disfunzionali e disadattive.
