The Ministerial Decree no. 77 of 2022 introduced significant organizational innovations in territorial healthcare, such as the Local Operational Centers for remote assistance (LOCs), aimed at improving care continuity between hospitals and primary care. The study examines the development of an integrated model for transitional care in the Lazio Region, focusing on the case of AOU Sant’Andrea and ASL Roma 1, Roma 4, and Roma 5, exploring the operational model, challenges, and preliminary outcomes, highlighting the crucial role of LOCs in ensuring a smooth and coordinated care pathway. Early results show
improvements in reducing hospital stay times and increased patient satisfaction, presenting a replicable model for national implementation.
